A two-time victor of the Academy Award for Best Director, the "One Flew Over the Cuckoo's Nest" and "Amadeus" helmer also won three Golden Globes, the Cannes Film Festival's Grand Prize of the Jury (for "Taking Off"), the Golden Bear at Berlin ("The People vs. Larry Flynt"), a BAFTA award, and numerous other accolades. One Flew Over the Cuckoo's Nest (1975), adapted from Ken Kesey's 1962 novel, dealt with life inside an OR mental institution.
He passed away last night but the cause is not yet known.

In a 2004 interview with the LA Times, Forman said, "When we started to make our films, they were really Czech films about Czech society and Czech little people - and who cares about Czech little people?" The Czech native was 86 years old. Forman then had another huge success with Amadeus, a film version of Peter Shaffer's play about the rivalry between Mozart and Salieri; it won eight Oscars, including Forman's second for best director.
Born to a Protestant family in the central Czech town of Caslav on February 18, 1932, the double Oscar winner's life was indelibly scarred by World War II.
His manager, Dennis Aspland, confirmed that he had been told by Forman's wife Martina that the director passed away on Friday 15 April after a short illness.
When Forman was honoured with a lifetime achievement award from the Directors' Guild of America in 2012, the guild's president Taylor Hackford said he "finds the universality of the human experience in every story".
Never prolific, Forman's output slowed even more after "Amadeus", and his three subsequent films were disappointments.
