In response, the Arizona Department of Economic Security and Phoenix police launched an investigation into the facility.
"Today, the San Carlos Apache Tribe (the "Tribe") learned of the tragic circumstances surrounding [redacted], an enrolled member of the Tribe".
"On behalf of the tribe, I am deeply shocked and horrified at the treatment of one of our members", tribal chairman Terry Rambler said.
"Trust has been severed, period", she said. "It is my hope that justice will be served".
A woman whose 22-year-old daughter is a patient at the facility said she refuses to leave her side until the investigation is conclusive.

The woman, who was incapacitated in a drowning incident, was a patient at Hacienda Healthcare when she went into labor on December 29 and delivered a baby boy, according to local media.
"I can't believe someone would bathe her daily for nine months, never know she wasn't having her period, she wasn't growing in her mid-section", one of the woman's former caregivers told ABC15, which did not reveal his or her identity.
Gary Orman, Executive Vice President of the Hacienda Board and a Board Member since 1992, vowed that Hacienda "will accept nothing less than a full accounting of this absolutely horrifying situation, an unprecedented case that has devastated everyone involved, from the victim and her family to Hacienda staff at every level of our organization".
Alejandro Benally, Chief of the San Carlos Apache Police Department said, "At this point, this matter falls under the jurisdiction of the Phoenix Police Department". "And we will do everything in our power to ensure the safety of every single one of our patients and our employees".
"I wasn't there. I clearly don't have firsthand knowledge of what happened", Meyers said.
Hacienda HealthCare was founded five decades ago.
He declined to say whether its staff were being asked to undergo DNA testing to identify a possible suspect, or whether the facility was taking any preventative measures to protect patients against a similar situation.
The facility provides intermediate care for patients with intellectual disabilities.
One staff member was sacked for making repeated inappropriate sexual comments to patients.
